Mission

Our mission is to provide our clients/residents a positive experience in a friendly environment. We provide the joy of living along with the supplemental care necessary for individuals who are ventilator dependent and those requiring 24-hour specialized nursing care. We focus on the individual strengths and needs of our residents. We strive to satisfy each individual's specific needs and concerns with personal dignity.

Licensed Staff

We provide 24 x 7 Nursing. Our staff members include a Registered Nurse (RN, Licensed Practical Nurses (LPNs), and Vent Care Assistants (Certified Nursing Assistants).

Licensed Facility

Autumn Grace is a Foster Care Home licensed by the State of Minnesota, Department of Human Services.

Staff Training

All licensed staff are trained annually on:

  • Ventilators
  • Tube feeding
  • Tracheostomy care
  • Suctioning
  • Bowel and incontinence programs
  • Medication administration
  • CPR/First Aid
  • Behavior management
  • Communication techniques
  • Personal care
  • Provision of considerate and compassionate care
  • Other training as needed per resident needs

Snacks and Meals

Meals are prepared with the individual dietary needs and/or restrictions per Doctor's orders as well as likes and dislikes. Meal adaptations include, but are not limited to:

  • No added salt (NAS)
  • Low cholesterol
  • Diabetic
  • Weight control
  • Tube feeding

Community Involvement

2009 ALS walk
Grace Senior Services, Inc. participated as the 'papa joe's pALS' team in honor of Joseph Winter, a resident of the Autumn Grace Foster Care / Vent Unit. Joe has suffered with ALS since being diagnosed in 1996. The Autumn Grace team raised over $3,000 of the $41,550 total raised during the Mankato walk. The total amount raised across Minnesota and North Dakota was $554,000.


The next ALS walk will be held in mid April 2010. If you are interested in walking with the papa joe's pALS team 2010 team, contact Heather Bass at 507-388-0645. Visit the Minnesota ALS Association for additional information or to join a support group.
